It only cost $400 to sponsor a patient for a full year & this is what you will provide your Sponsored Patient through the CCClinic:

Medications with dose adjustments as needed

Lifestyle Modification Education, goal setting & accountability

Glucose strip & supplies to continue monitoring their glucose at home

On-going laboratory appointments to monitor their clinical progress

Transportation to laboratory appointments

We travel to each patient & make house visits as needed

Our goal is to expand to include services for

High Blood Pressure & enroll new patients to the program!
